Safety in Healthcare

In November 1999, the Institute of Medicine (IOM) issued a safety report, "To Err is Human", which estimates that 2.9-3.7% of US hospitalizations result in adverse events, and 44,000-98,000 hospitalized patients die annually due to medical errors.

 In addition to medical errors there are other safety considerations that impact staff and individuals who are delivering and receiving care in healthcare facilities. Those safety considerations include protections from fires, falls or abuse just to name a few.

The following web site is intended to alert providers and consumers in Wisconsin about safety issues that have occurred in Wisconsin and nationwide.

In addition the ongoing goal of the website is to provide a gathering place of resources that providers may find helpful in creating a culture of safety. The web site attempts to gather a range of resources into one location.
