For complaints about a facility,
contact the Regional Office that oversees the county in which the facility is
located.
All fires in a licensed health or residential care facility in Wisconsin
must be reported to the Department of Health Services within
72 hours (check specific rule requirements).
You
may use the Fire
Report Form (PDF, 23 KB) and fax it to the appropriate Regional
Office or to (608) 267-7119 or call David Soens at (608) 261-5993.
