For information for management and staff of health care or residential care providers: Review List of entities regulated by the Division of Quality Assurance. Select the provider type you are interested in. You will access its introductory page.

Licensing and certification information for all DQA-regulated providers is available via wisconsin.gov:

  1. Select Licensing and Permitting listed under Quick Links.

  2. Go to the "Search by State Agency" area.

  3. Select "Department of Health and Family Services" from the drop-down list.

  4. Select the Search button in the "Search by State Agency" area.

This will display license and certification data for DHFS programs. You will find information on what licenses or certificate types are available, including, but not limited to:

  • probationary/continuing licenses or certificates
  • third-party payment (Medicare)
  • relevant fees
  • links to the appropriate DHFS (DQA) Licensing pages
  • telephone and E-mail contacts
