Wisconsin Self Directed Supports Waiver
Effective July 1, 2008 persons eligible for publicly funded long-term care
will have a new option available to them. The option which is currently known as
the Self Directed Supports (SDS) Waiver will be available in each county as that
county implements Long-Term Care Reform and Expansion. The person choosing
the SDS Waiver is not a part of managed care, and is known as the
"participant." Participants self manage their supports and services
with the help of others as requested.
In the Self Directed Supports (SDS) Waiver option, the state calculates a
budget for each individual, and an independent consultant works with the person
to create an individual support and service plan. Plans are approved by the
consultant, and may include supports or items that help the
participant achieve his/her own outcomes. One service option is Participant
Customized Goods and Services (PDF 18KB). In the SDS Waiver a financial services agency
pays the participant selected providers that are listed on the individual
support and services plan.
