Person-Centered
Planning
Person-centered planning technologies are used throughout Wisconsin to help determine
the interests and preferences of people with developmental disabilities. The supports are
developed in response to the identified interests, needs and preferences to help people
get lives that make sense. People are trained to facilitate and do training in MAPS,
Personal Futures Planning, Essential Lifestyles Planning and PATHS throughout the state.
For more information about person-centered planning
(Exit DHS) and related supports contact the Bureau of Long-Term Support at (608)
