How to get materials

Below, you will find links to helpful materials that can be printed directly from this site. 

Some of these items are available in paper and can be ordered completing a request form.  After completing the form, email it to: DHFSFMHCFPHC@wisconsin.gov for processing. Most orders are sent out within 3 days.

Those items available in paper format are indicated with an asterisk (*) after the publication/form number (in bold below).
